16-Week Back Pain Rehabilitation Programme

A structured physiotherapy-led rehabilitation programme for persistent back pain.

The Four* Spine Programme supports people who experience recurring flare-ups, reduced confidence in movement, or ongoing back pain that hasn’t improved with standard treatment.

Unlike drop-in exercise classes, this is a structured programme of care overseen by Advanced Practice Physiotherapists with specialist NHS spinal expertise.

Why a Programme Instead of a Class?

Persistent back pain often requires:

  • Time and structured progression
  • Education and reassurance
  • Gradual rebuilding of strength and confidence

The programme combines:

  • Supervised progressive exercise
  • Small group clinical oversight
  • Education on managing flare-ups
  • Long-term self-management strategies

Programme Structure

The programme includes:

  • Initial 60-minute physiotherapy assessment
  • 16-week structured rehabilitation programme
  • Two supervised sessions per week
  • Small groups (maximum 6 participants)
  • 4–6 education sessions

Who Leads the Programme?

The programme is led by Advanced Practice Physiotherapists with senior NHS spinal expertise.

They have extensive experience managing complex spinal conditions, working with surgical teams, and supporting people with persistent back pain.

This level of clinical expertise is rarely available outside hospital services.

Who the Programme is Not For

  • Acute back pain less than 6–8 weeks old
  • Severe pain preventing exercise
  • Progressive neurological symptoms
  • People requiring urgent spinal assessment
